Add name and/or phone number to "Request Support"

Need a name to identify the user as we have several generic users. If this could be an option to turn on mandatory name/number, this would force users to give me a name that I can contact.

We use this feature to generate an email to one of our clients ticket systems. it would be awesome if they put in their name and email and then the email would come from them. It would prevent us from having to dig to find who it is and change the name in the ticket sys.
